Clinical Summary: Reframing Atopic Dermatitis as a Systemic Condition and Enhancing Cross-Specialty Care
Clinicians managing atopic dermatitis (AD) are urged to actively counter pervasive patient and caregiver misconceptions and adopt a collaborative, cross-specialty paradigm for comprehensive disease management. Adam Friedman, MD, FAAD, professor and chair of the Department of Dermatology at the George Washington University School of Medicine and Health Sciences, highlighted the necessity of steering patients and their families toward evidence-based therapeutic strategies.
Dr Friedman underscored that optimizing outcomes requires clinicians to recognize and communicate the broader pathological scope of AD beyond its cutaneous manifestations. Clinicians are encouraged to prepare standardized talking points to clearly convey the systemic nature of the disorder and its associated comorbid risks during clinical encounters. Regarding this communication approach, Dr Friedman stated:
“Understanding the comorbidities of atopic dermatitis, having a script in your head of how you talk about these to your patients so they understand that this is truly a systemic disease, will really allow you to have a more efficient and, hopefully, more successful conversation when it comes to long-term management strategies.”
Because AD carries significant systemic implications and multiorgan comorbidities, managing the disorder effectively demands close coordination between dermatologists, primary care providers, and relevant allied medical specialties.
Table 1. Core Clinical Principles for Long-term Atopic Dermatitis Management
| Domain | Key Recommendation / Actionable Approach |
| Disease Conceptualization | Recognize and explain atopic dermatitis as a chronic, systemic inflammatory disease rather than an isolated cutaneous pathology. |
| Patient & Family Guidance | Counter common misperceptions through structured, evidence-based counseling focused on realistic long-term outcomes. |
| Communication Strategy | Develop a structured mental framework or “script” to efficiently educate patients regarding potential comorbidities and maintenance therapies. |
| Interdisciplinary Coordination | Establish collaborative referral networks across medical specialties to address systemic co-occurring conditions comprehensively. |
